Check eligibility and find the agency officer

The statutory requester definition includes a person legally resident in the United States. Check that definition before making an eligibility statement. Pennsylvania: OOR statutory copy

The OOR directs requests to the agency likely to hold the records and its Agency Open Records Officer. Its instructions offer an officer directory and the official Standard RTK Request Form, which can be used for Commonwealth and local agencies. Pennsylvania OOR: requests and official form

Use the official form route and a specific records description

Adapt the text below into the official OOR form or the agency's accepted channel. The OOR lists email, fax, mail and in-person submission, and stresses that the law concerns records rather than answers to questions. Keep the submitted request. Its own online OOR-records form is only for records held by that office. Pennsylvania OOR: requests and official form

Track the written response and any extension

The Citizens' Guide describes five business days for a written grant, denial or valid extension. It counts from the day after receipt during regular business hours. A failure to respond within the allotted time can be a deemed denial. Pennsylvania OOR: response and fee guide

Section 902 requires a timely extension notice stating the review reason, a reasonable response date and estimated applicable fees. A date beyond 30 days after the initial five business days produces a deemed denial unless the requester agrees in writing to the specified extension. Pennsylvania: OOR statutory copy

Ask about fees before agreeing to charges

The OOR's guide says legal-review and redaction time cannot be charged. It identifies possible prepayment when expected fees exceed $100. Ask for an estimate and follow the current fee schedule for the format requested. Pennsylvania OOR: response and fee guide

Copy and adapt the records description

This is adaptable request text, not a replacement for the official form or the chosen agency's accepted process. Replace every bracketed field, use truthful eligibility information and narrow the log period. This site does not submit it.

Choose the appeal route for the records involved

For an ordinary OOR appeal, its filing guide gives 15 business days from the mailing date of the response or the deemed-denial date. Include the original request, response or deemed-denial explanation, reasons the records are public, and an answer to each denial ground. Its online appeal form guides the filing. Pennsylvania OOR: appeal filing

Section 503(d)(2) assigns appeals concerning local-agency criminal investigative records to an appeals officer designated by the county district attorney. Other listed bodies also have designated officers. Check the applicable jurisdiction rather than treating every police-record dispute as an ordinary OOR appeal. Pennsylvania: OOR statutory copy

Use the separate process for law-enforcement recordings

The OOR says Act 22 governs requests for law-enforcement audio and video recordings and that RTKL does not apply to those requests. It publishes a separate guide and form. The governance-document template here does not request recordings. Pennsylvania OOR: separate police-recording process
